چکیده

OBJECTIVES To describe a technique for surgical removal of recurrent respiratory papilloma in the distal trachea in patients with an indwelling tracheostomy tube. METHODS A transoral trans-stomal technique for removal of distal tracheal papilloma using a modified ventilating bronchoscope setup and a microdebrider with a rotatable Skimmer blade (XOMED Products, Jacksonville, Florida, USA). RESULTS Surgical removal of papillomas in the distal trachea is a challenging procedure, involving potentially competing demands for visualization, instrumentation, and ventilation. Previously described methods for management of these challenging cases provide limited ability to deliver continuous oxygenation and ventilation during the procedure. The transoral trans-stomal technique overcomes these limitations in patients who have an existing tracheostomy. CONCLUSIONS This technique allows for precise removal of papilloma in the distal trachea, while providing for excellent visualization and continuous oxygenation and ventilation of the patient.

Treatment of recurrent respiratory papillomatosis in children with the microdebrider.

The mainstay of treatment of children with recurrent respiratory papillomatosis has been CO2 laser laryngoscopy. Powered microdebriders allow precise removal of laryngeal lesions without thermal damage. The microdebrider may reduce costs by reducing operative time and limiting the need for expensive laser-related equipment. To compare the use of the microdebrider with that of the CO2 laser in c...

Cidofovir injection for recurrent laryngeal papillomatosis.

BACKGROUND Recurrent laryngeal papillomatosis is caused by the human papilloma virus and characterized by multiple exophytic lesions of the vocal tract; it is a benign disease, but one that tends to recur after removal of growths. Due to the nature of the disease, alternative treatment strategies have focused on antiviral injection at lesion sites that eliminate repeated surgical procedures. Th...

Tracheal stricture following tracheostomy.

Six cases of tracheal obstruction following tracheostomy are described in which surgical intervention was necessary. These occurred in a series of 104 tracheostomies. In adults the strictures have occurred at the site of pressure of the tracheostomy tube cuff. Management has been by excision of the stricture with end-to-end reconstruction. Tracheal narrowing in children has been of a different ...
